UP CPAT Application Form 2017:

MGKV (Mahatma Gandhi Kashi Vidyapith), Varanasi is the main conducting authority of the CPAT (Combine Pre Ayush Test). CPAT exam is conducting for those applicants who are wishing to get admission into BAMS, BUMS, and BHMS programmes. UP CPAT Application form 2017 is expected to be available in the month of September, 2017.

Instructions to Apply for UP CPAT 2017:

  • In order to fill UP CPAT Application form 2017, applicants firstly required to visit the official website of the exam conducting authority i.e. CPAT.
  • For applying online, you must have an internet-enabled computer system and scanned copy of your photograph, signature and right thumb impression.
  • First of all applicants need to click the registration/login link and accept the Declaration of the applicants.
  • Now complete the registration with information such as your name, father’s name, mother’s name, category, date of birth, email ID and your mobile number.
  • Thereafter, a user ID and password will be sent to your registered email ID and mobile number.
  • Now, you need to click the link for making fee payment available at Application Status page.
  • After paying the required fee, you need to submit your form.
  • Application Fee can be paid by using your debit card/credit card/net banking. Otherwise, one can also submit the fee at any Allahabad bank branch in cash.
  • Now upload your photograph, right thumb impression and signature.
  • After uploading the aforesaid documents, applicants need to click the Final Submit button.
  • After submission of the form, applicants should take printout of their filled form.
  • Applicants are restricted to submit their applications before the notified deadline, otherwise the same will be rejected.

Scanned Photograph

  • Signature must be in .jpg format only.
  • Don’t use any special character while saving your photograph.
  • Photograph must be in prescribed manner otherwise form will be rejected.
  • Applicant must use coloured photograph in their application form.
  • While snapping the photograph applicant need to write their name, day and time of snapping the photograph on the piece of paper. And place the paper below their neck so that it will be clearly printed on your photograph.
  • Applicants need to make minimum 10 (passport size) photocopies of that image which will be further used in Admit card, Exam, Counselling etc.

Photograph must be colored along with 3.5 X 4.5 cm of size. If applicant wear black glasses, cap then their form will not be accepted.

Don’t use Polaroid camera or ink jet printer to click the photograph as it is not considered.

Applicant must note that scanned photograph must not be older than 1st of March, 2016.

Right Hand Thumb Impression

  • Applicant need to attach a Right Hand Thumb impression snap in the application form.
  • Image must be in .jpg format only.
  • Don’t use any special character while saving your photograph.

Scanned Signature:
